 Instructions:  Fill out the entire form, checking appropriate boxes or filling in lines. Any incomplete forms may result in a delay in getting the event approved. Requests need to be made at least 30 days prior to the event.
All events are subject to approval by the Governing Board of the Arena Congregational Church.

We will need the building locked and unlocked for us at above noted times

We will provide our own clean up and will leave the premises as clean as we found them.

Clean-up includes putting tables back in their original locations, putting away any additional tables that were put out, emptying of any full trash bags and disposing of them, sweeping or vacuuming of any floor soiled, washing of soiled tables, dishes, or other items used.

We request Custodial Services at an additional fee.

We will require ongoing storage space at an additional fee.

We will have electrical appliances or equipment used on a regular basis.

We will need equipment set-up, i.e. tables and chairs.
